As a condition of including this form on the Illinois State Bar Association (ISBA) Form Exchange for no monetary compensation, you hereby grant the ISBA permission to: covert the form to the proper electronic medium for posting on the forms center; attach your name to the form as the form provider, the name of the form, the date the form was submitted to the Form Exchange, and the date the form was created/updated; and, attach the following disclaimer:

As a form provider, you agree to indemnify and hold harmless the Illinois State Bar Association from any and all claims, suits, actions or proceedings of any kind arising out of, or in connection with, participation in the Illinois State Bar Association Form Exchange.

Notice:

If you are a form provider found to have infringed upon the copyright of others, the ISBA may in appropriate circumstances pursuant to 17 U.S.C. § 512(i)(1)(A), terminate a members access to the Form Exchange and otherwise decline or remove postings from that member. As a user of this service, you understand and acknowledge that the ISBA does not monitor, review, revise or update the forms posted. The ISBA may remove any form for any reason at any time without notice to the form provider. The ISBA will remove a form at the request of the form provider.
