Alliance Of Bar Associations For Judicial Screening
Dear Judicial Candidate:
Members of the Alliance of Bar Associations for Judicial Screening invite you to participate in our evaluation and interview process.
You will find the questionnaires that the Alliance utilizes in its investigative process. The format of these questionnaires is in MSWord and is designed to be used with your personal computer. Please note the following:
- No handwritten questionnaires will be accepted;
- All Alliance deadlines of submission of applications must be adhered to;
- Candidates are to submit two (2) diskettes in MSWord form and twenty (20) hard copies of completed application;
- The candidate is responsible for the completeness and accuracy of the information provided in response to this questionnaire;
- Telephone numbers for references must be current;
- Any changes in residence or employment must be noted, in writing, immediately;
- The candidate has an ongoing duty to notify the Alliance of any corrections and/or updates to the information provided in this questionnaire;
- If the candidate has any question or doubt about the meaning of any particular question or whether to disclose a particular fact or circumstance, s/he should contact the Alliance Administrator;
- No question should be construed as requiring violation of Illinois Supreme Court Rule 67, which prohibits indicating a position on a matter that may come before a judge.
- Please also submit one executed copy of the Full Disclosure Statement, Candidate Evaluation Agreement, and Authorization and Waiver.
- The completed materials are due by October 12, 2009 unless otherwise noted.

Puerto Rican Bar Association of Illinois, and Women’s Bar Association of Illinois,
working collaboratively to improve the process of screening judicial candidates in Cook County, Illinois.
